UPMC Home Healthcare's Diabetic Program was developed to manage patients with this chronic disease in their own home. With adequate education, medication management, and multi-disciplinary care, patients with diabetes can be provided with appropriate and timely interventions to avoid preventable complications and hospitalizations.

Goals of the Diabetes Program
  • Detect signs of uncontrollable diabetes and initiate appropriate intervention
  • Timely intervention to reduce re-hospitalizations and/or Emergency Room visits
  • Improve patient compliance with treatment plan
  • Improve quality of life through individualized patient education regarding disease management
  • Promote patient self-management to delay onset and decrease severity of complications
  • Facilitate effective communication between patient, clinician and physician
