Healthy Hearts at Home

Heart Failure and Chronic Obstructive Pulmonary Disease (COPD)

Healthy Hearts at Home is a program for patients primarily diagnosed with heart failure but also can be for patients with conditions such as coronary artery disease, hypertension, myocardial infarction, and COPD. Specialized nurses are able to closely monitor these patients and report significant changes to the attending physician to provide the appropriate interventions in the home. Our Connected Care program has also been a successful way to provide closer supervision and support.

Diabetes

UPMC Home Healthcare's Diabetes Program was developed to manage patients with this chronic disease in their own home. With adequate education, medication management, and multi-disciplinary care, patients with diabetes can be provided with appropriate and timely interventions to avoid preventable complications and hospitalizations.

Specialized Wound Care

A multidisciplinary team, led by two wound care specialists, partners in wound care management with the attending physician and provides treatment according to protocols based on industry standards and most current research. Learn more >>

Maternal-Infant

The Maternal-Infant program provides specialized nursing visits for high risk ante-partum care, early maternal and infant discharge and mother and infants with identified risks and complications post-partum. Learn more >>

Pediatric

The Pediatric Program provides care to children from birth through age 18 and includes cardiac assessment, feeding and growth assessment, wound care, and post -surgical care. High tech care includes central line care and infusion therapy such as Total Parenteral Nutrition (TPN), enteral feedings, antibiotics, and inotropic medications. Our experienced registered nurses also provide chemotherapy at home after specialized education and certification.

Infusion/Oncology

Our Infusion/Oncology program is led by a masters prepared Oncology Specialty Clinician. Our staff attends advanced IV education as well as the Foundations to Oncology, Comprehensive Chemotherapy and Stem Cell transplant classes at UPMC Cancer Centers. Learn more >>
