 PROGRAMS:  FALL PREVENTION
Fall Prevention

All of our patients get the benefit of our Fall Prevention Program. We will check your home for anything that could put you or others at risk for a fall and develop a Falls Risk Assessment including a patient's history of falls, ability to ambulate, vision, gait, balance, and home safety evaluation.

Goals

  • To assess and improve balance during gait, and reduce fall risk in older adults in their home environment
  • Assist resident in functioning independently and safely in the home
Our specially-trained physical therapists will:
  • Systematically examine each patient's impairments, functional mobility and gait
  • Assess balance function using standardized balance assessment tools
  • Determine fall risk based on standardized screening tools
  • Provide treatment for each patient/resident according to problem-based needs
  • Address safety issues within the home environment
  • Recommend appropriate equipment, if necessary
  • Provide and instruct in a home exercise program
