 PROGRAMS: HEALTHY HEARTS AT HOME
Healthy Hearts at Home follows your doctor's plan to help you recover at home. A nurse with special training in heart and lung problems will get the orders from your doctor about the plan for care. Then he or she will make visits to your home and will stay in touch with your doctor as you progress.
Healthy Hearts at Home helps patients to:
  • take their medicines properly
  • understand diet and fluid needs
  • get more information from our registered dietician
  • keep track of their weight
  • maintain a healthy activity level
  • recognize serious symptoms
  • know when to call the doctor
Is this program for you?
Healthy Hearts at Home is for patients with:
  • Congestive Heart Failure
  • COPD (chronic obstructive pulmonary disease, which includes emphysema and chronic bronchitis)
  • Asthma
  • Other Heart and Lung Diseases
Care when it's needed
We are fully staffed seven days a week for home visits. We make home visits on weekends and holidays when necessary. After normal business hours, a registered nurse is always available to take calls.
